Request for duplicates

In the event of loss of national service documents (exemption card or military or suspended booklet), the person concerned must report to the Consulate General……. to request a duplicate, with the following documents:

a handwritten request for a duplicate.
A declaration of loss issued by the consulate.
The full copy of the birth certificate (transcribed certificates for people born abroad).
A copy of the consular card.
Two (02) regulatory and recent passport photos.
